CONFLICT DISCLOSURE STATEMENTS

Effective January 1, 2006, members of a municipal governing body are required to file Conflict Disclosure Statements with their City Records Administrator under the provisions of House Bill 914 and Local Government Code Chapter 176. A Conflict of Interest Questionnaire shall be filed by certain vendors and other persons that might have conflicts of interest with a local government entity under the provisions of H.B. 914.

This questionnaire is being filed in accordance with Chapter 176, Local Government Code by a person who has a business relationship as defined by Section 176.001(1-a) with a local governmental entity and the person meets requirements under Section 176.006(a).

Conflict of Interest Questionnaire (CIQ)

This is the notice to the appropriate local governmental entity that a local government officer has become aware of facts that require the officer to file this statement in accordance with Chapter 176, Local Government Code.
